What a split sheet template should include
Song details: the song title, the date it was written, and optionally where it was written and who the recording artist is.
For every writer: their full legal name, their percentage of the song, their role (writer, producer, composer or lyricist), their performing rights organization (ASCAP, BMI, SESAC, GMR or an international society), their IPI number, and their publisher if they have one.
Signatures: a signature and date line for every writer, plus a statement that everyone agrees to the percentages listed.
Why a fill-in template beats a PDF form
Most free split sheet PDFs are static forms. The most common mistake on them is shares that don't add up to exactly 100%, which causes problems when songs get registered. A fill-in template catches that before anyone signs.
